General Insurance Information

MDR is an approved provider for most major insurance carriers and health plans. We will submit your claim, and any secondary Picture of a MRI computerclaims, to any insurance company you specify, providing we participate in that specific insurance program.

It is important to take the following steps before your appointment:

(1) Determine if your plan requires pre-certification for the study you need to have performed. An MDR scheduling or billing representative will provide you with this information if necessary. If you need pre-certification, contact your referring physician and he or she will obtain it for you and forward it to us on your behalf.

(2) Give MDR complete insurance information upon your arrival, including your insurance card for photocopying. You will be asked for your policy ID and group numbers, the name of the policy holder, and the claims address of your insurance company (all listed on your card).

Ultimately, the patient is responsible for payment, including any balance not covered by insurance. We will send you a balance statement for any amount you might owe after insurance payment is received.
